Use of Facility Agreement

I, the undersigned parent or legal guardian of the participant named above, understand that Dynamic Taekwondo, Inc. is allowing the use of its facility for cheerleading and related training activities free of charge.I acknowledge and agree to the following:

1. Participation in athletic activity carries inherent risks, including but not limited to physical injury, illness, or property damage.

2. I voluntarily allow my child to participate and use the facility at my own risk.

3. I release, waive, and hold harmless Dynamic Taekwondo, Inc., its owners, instructors, employees, and agents from any and all liability, claims, demands, or causes of action arising out of or related to any injury, illness, or loss that may occur while using the facility.

4. I understand that Dynamic Taekwondo is not responsible for providing medical insurance or coverage for injuries or accidents that occur while using the facility.

5. I am responsible for any medical expenses that may result from participation.

6. All participants are expected to follow on-mat rules and maintain proper behavior while in the facility.

7. Only approved competition shoes are permitted on the mats. No street shoes, cheer shoes, or other footwear may be worn on the mats at any time to prevent damage.

8. The facility and its equipment (including mats, mirrors, and training gear) must be treated with care and respect.

9. Any damage caused by misuse, neglect, or failure to follow rules may result in repair or replacement costs billed to the individual, team, or organization responsible.

10. Dynamic Taekwondo reserves the right to deny or revoke facility access at any time if safety rules or conduct standards are not met.
